Comments:
An unannounced monitoring inspection was conducted on June 11, 2019 from approximately 10:00 am to 12:10 pm to review subsidy health and safety inspection requirements. Provider was present with one child for a total of four points. Provider and household member records were reviewed. Five children's records were also reviewed.

Violations:
Standard #: 22VAC40-665-160-B
Description: Based on observation and review, provider did not ensure there is a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was one child present during inspection and he was not noted on the attendance sheet.

Plan of Correction: Will put his name in the book.

Standard #: 22VAC40-665-370-E
Description: Based on interview and observation, provider did not ensure the diapering surface is nonabsorbent. Evidence: Provider stated she uses a sheet over the diapering pad when changing children's diapers.

Plan of Correction: Will not use a sheet on the pad.

Standard #: 22VAC40-665-400-B-7
Description: Based on review and interview, provider did not ensure required information was documented in the emergency preparedness plan. Evidence: Provider did not have documentation of the continuity of operations procedures to ensure that essentials are maintained during an emergency.

Plan of Correction: Will add the essential functions procedures to the emergency plan.

Standard #: 22VAC40-665-460-C
Description: Based on interview, provider did not ensure when children stay overnight that each child sleeps in a bed with a mattress or a cot with at least two inches of dense padding. Evidence: Provider stated that the children that stay overnight sleep on the cots with blankets. No dense padding is available on the cot.

Plan of Correction: Will order mats today to use on the cot.
