Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program. A monitoring inspection was initiated on and concluded on 7/27/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 2 years old to 9 years old, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 caregiver/household records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-111-800-B
Description:	Based on review, the provider did not review the emergency plan at least annually and update the plan as needed. Evidence: 1. The emergency plan was last reviewed on 12/19/2019.
Plan of Correction:	I will do it today.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, children's records did not all contain required information. Evidence: 1. Child #1 did not have previous child care centers. 2. Child #2 did not have previous child care centers and the medical insurance policy number.
Plan of Correction:	I asked the parents for the information.
Standard #:	22VAC40-111-180-A
Description:	Based on interview, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years. Evidence: 1. The provider stated caregiver #1 had not received an updated TB test or screening. Last date of test or screening was on 5/17/2019.
Plan of Correction:	I have scheduled one for 8/17/2021
Standard #:	22VAC40-111-210-A
Description:	Based on review of training, the caregiver did not obtain a minimum of sixteen clock hours of training annually. Evidence: 1. The provider had a total of 6 1/2 hours of training in 2020.
Plan of Correction:	I will work on completing the training for 2020 and complete this years training.

Description:	Based on interview and observation, the provider did not have the most recent inspection posted for others to view. Evidence: 1. The provider did not have the 12/11/2020 inspection posted.
