This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A monitoring inspection was initiated virtually and concluded on Thursday, August 5, 2021.The provider was contacted by telephone to initiate the inspection. There were 11 children present and 3 caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver and 1 household member's records, along with any requested program records submitted by the facility to determine if required
documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 22VAC40-111-140-B-3 Description: Based on review of caregiver's record and interview, the provider did not ensure to obtain a current certification in cardipulmonary resuscita.
Evidence: In Caregiver's #1's record, the certification in cardiopulmonary resuscita was due for renewal on 10/2020. Interview with provider confirmed this was the most recent certification available for review during this inspection.Plan of Correction: The provider will call to schedule training for first aid and cardiopulmonary resuscita today.
Standard #: 22VAC40-111-140-B-4 Description: Based on review of caregiver's record and interview, the provider did not ensure to obtain a current certification in first aid.
Evidence: In Caregiver's #1's record, the first aid certificate was due for renewal on 10/2020. Interview with provider confirmed this was the most recent certification available for review during this inspection.Plan of Correction: The provider will call to schedule training for first aid and cardiopulmonary resuscita today.
Standard #: 22VAC40-111-180-A Description: Based on review of caregiver's record and interview, the provider did not ensure to obtain a current Report of tuberculosis screening form, every two years from the of the first screen or more frequently as recommended by a physician or the local health department.
Evidence: In Caregiver#1's record, the last tuberculosis screening was dated 04/10/19. Interview with the provider confirmed this was the most recent screening.Plan of Correction: The provider will schedule appointment for tuberculosis screening today and include results in record once received.
