Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on August 9, 2021 and concluded on August 9, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were four children present, ranging in ages from 5 months to 4 years, with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on documentation review and interview, the provider did not obtain a tuberculosis (TB) screening every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
The record for Provider #1 contained a TB screening dated 07/25/2018. Provider #1 stated she has been having difficulty getting an updated TB screening.

Plan of Correction: I have been having difficulties scheduling a doctor's appointment. I will reach out to the doctor and find a location that will do an updated TB screening.

Standard #: 63.2-1721.1-A
Description: Based on documentation review and interview, the provider did not, the provider did not obtain all required updated background checks.
Evidence:
The record for Provider #1 contained a Virginia central registry finding dated 01/21/2016. Central registry findings are required to be updated every five years from the date on the previous form.

Plan of Correction: I did not realize it was time to update the central registry form. I will send off the request for the updated background check.
