Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program. A monitoring inspection was initiated on July 21, 2021, and concluded on July 22, 2021, approximately 10:44 am to 11:15 am. The director was contacted by telephone and a virtual inspection was conducted. There were 7 children present, ranging in ages from infants to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	Based on staff records review, 1 out 2 records, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: Staff #1 Tuberculosis Screening expired on 12/21/2018.

Description:	Based on staff records review, 1 out 2 records, the provider did not have documentation of every 5 year repeated central registry check.
