This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on September 14, 2021 and concluded on September 15, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were three children present for a total of nine points, ranging in ages from two years to four years of age, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and two caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22VAC40-111-780-B Description: Based on review of five children's records, the provider failed to obtain a parent's written acknowledgement of review of emergency information annually.
Evidence:
1. Child #5's record had no written documentation of a review of emergency information with the parent. The last date reviewed was 02/25/2020.
2. Child #6's record had no written documentation of a review of emergency information with the parent. The last recorded date of review was 04/27/2020.Plan of Correction: The provider will contact the parents this evening at pick-up and have them complete a review of emergency contact information.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, the provider failed to obtain an updated tuberculosis screening every two years from the date of the first screening.
Evidence:
1. Caregiver #1's last documented tuberculosis screening was dated 12/11/2018.
2. Caregiver #2's last documented tuberculosis screening was dated 09/27/2018.
3. The provider verified that updated tuberculosis screening had not been obtained.Plan of Correction: The provider will make appointments for all caregivers to obtain TB screening this week.
