One would need both the facility identification number and the Fieldprint Code number to schedule an appointment to obtain this finding. Comments:
A monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 09/24/2021. The Provider was contacted and a virtual inspection was conducted. There were 4 children present (9 points), ranging from 20 months to 3 years in age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition (meals and snacks brought from home). A total of 4 children records, 1 caregiver record, and 2 household member records were reviewed.

Information gathered during the inspection determined that there were non-compliances with applicable standards, code, and/or law. These violations were documented on the Violation Notice issued to the program.

Violations:
Standard #: 22VAC40-111-170-A
Description: Based on review and interview, 1 of 2 household member records indicated that the Provider did not obtain from each adult household member a current Report of Tuberculosis Screening (TB).

Evidence -
1. On the date of inspection (09/24/2021), TB documentation for Household Member #2 was not available for review.
2. The Provider stated (during an interview conducted on the date of inspection) that Household Member #2 is away and will not return until next week.
3. This requirement for Household Member #2 came into effect in mid August 2021.

Plan of Correction: Household Member #2 shall schedule an appointment in order to obtain TB documentation. Once this documentation is received, it shall go on file at the Family Day Home (FDH).

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 2 household member records indicated that the Provider did not obtain an updated Report of Tuberculosis Screening (TB).

Evidence -
1. On the date of inspection (09/24/2021), TB documentation for Household Member #1 was dated - 04/12/2019.
2. This finding was confirmed by the Provider on the date of inspection.

Plan of Correction: Household Member #1 has scheduled to get updated TB documentation on 10/13/2021. Once received, this documentation shall go on file within the Family Day Home (FDH).

Standard #: 22VAC40-191-40-D-4-a
Description: Based on review, 1 of 2 Household Member records indicated that a Sworn Statement or Affirmation form (SDS) was not available.

Evidence -
1. On the date of inspection (09/24/2021), a SDS was not available for Household Member #2.
2. It was also determined that this documentation was not requested within 30 days prior (in mid July) of Household Member #2 becoming 18 years of age (in mid August).

Plan of Correction: This documentation shall be completed and placed on file within the Family Day Home (FDH).

Standard #: 63.2-1721.1-B-2
Description: Based on review, 1 of 2 Household Member records indicated that a fingerprint check (FPC) from the Office of Background Investigations (OBI) was not available.

Evidence -
1. On the date of inspection (09/24/2021), a FPC was not available for Household Member #2.
2. It was also determined that this documentation was not requested within 30 days prior (in mid July) of Household Member #2 becoming 18 years of age (in mid August).

Plan of Correction: Household Member #2 shall schedule an appointment with Fieldprint of Virginia in order to obtain this fingerprint check. Once this finding is received, it shall be shared and reviewed by the Licensing Inspection (LI).
