Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program. A renewal inspection was initiated on October 7, 2021 and concluded on October 8, 2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 1 staff record was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice. Description:	Based on review of caregiver's and household member, the provider failed to obtain current Central Registry Background checks. Evidence: Caregiver #1 Household Member #2 and HHM#3's records do not contain documentation of a current Central Registry Background checks. Caregiver #1's Central Registry Check expired 5/9/2021 HHM #2's Central Registry Check expired 5/13/2021 HHM #3's Central Registry Check expired 5/13/2021

Description:	Based on a review of household member's records, the provider failed to obtain a Central Registry Background check within 30 days of the household members 14th birthday.
Plan of Correction:	I will obtain the Central Registry check for the 14 year old.
Standard #:	63.2-1721.1-B-2
Description:	Based on a review of household member's records, the provider failed to obtain fingerprints for household member over the age of 18. Evidence: Household member #2's record does not contain documentation of submitting to a fingerprint check.
