Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.
A renewal inspection was initiated on 10/25/2021 and concluded on 11/1/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 10 children (21 points) present, ranging in ages from 18 months to 4 years, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 child records and 3 caregiver records, and 1 household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact me at shahana.green@doe.virginia.gov.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on a review of records, the Provider did not obtain a report of tuberculosis screening every two years from the date of the first screening.

Evidence:
1. Caregiver #1's most recent tuberculosis screening was dated 8/16/2019.
2. Household Member #1's most recent tuberculosis screening was dated 8/14/2019.

Plan of Correction: We will get new ones tomorrow. We have made an appointment.

Standard #: 63.2-1720.1-B-4
Description: Based on a review of caregiver records, the Provider did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. During the inspection, Caregiver #3 (doh: 10/25/2021) did not have a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from State #1 in which the individual has resided in the preceding five years.

Plan of Correction: I will work on doing these.
