Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 11/5/21 and concluded on 11/10/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 daycare children and 1 household child present with the provider, for a total of 6 points. A total of 2 children's records and 2 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Description:	Based on record review and interview, the provider did not ensure that children's records contain all the required information. Evidence: 1. The record for child 1 does not contain the addresses of two designated persons to contact in case of an emergency if the parent cannot be reached. 2. The provider confirmed that child 1's record lacks the addresses for the two emergency contacts.
Plan of Correction:	The provider responded with the following: Addressed for child 1's emergency contacts will be obtained from the parent.
