Comments:
An on site monitoring inspection was conducted on November 18, 2021. The provider was present and assisted with the inspection. There were four children present, ranging in ages from two years to four years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, two caregiver records and three household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not maintain arears and furnishings of the family day home, inside and outside, in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin.

Evidence:

The wooden privacy fence around the rear yard used by children in care, has multiple boards that have detached from the post exposing a nail. Additional board have split creating a jagged edge that could snag the skin of a child.

Plan of Correction: Per the provider: We have contacted someone to give us an estimate to repair the fence. We will have the fence repaired and keep the children away from it until the work is complete.
