Technical Assistance:

Technical assistance was offered regarding: children's records, background checks, and upcoming license renewal.

Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 11/16/21 and concluded on 11/30/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children (14 points) present, with 2 caregivers supervising. The children were observed playing with toys, watching children's programs on television and preparing for lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, special care and emergencies and nutrition. A total of 3 children's records and 2 caregiver's records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22VAC40-111-60-B
Description:	Based on records review, the provider did not maintain a complete record for each child in care. Evidence: Child # 1's authorization for emergency medical care was not signed and dated by the parent.
Plan of Correction:	I will have the parent sign and date it.
Standard #:	22VAC40-111-170-A
Description:	Based on record review, the provider did not obtain from each caregiver, at the time of hire a current Report of Tuberculosis (TB) Screening prior to coming into contact with children. Evidence: 1. Caregiver # 1's first day of hire was 11/1/21. 2. The provider did not obtain a TB screening for Caregiver # 1 prior to the caregiver coming into contact with the children.
Plan of Correction:	A negative TB screening was obtained.
Standard #:	22VAC40-191-60-B
Description:	Based on records review and staff interview, the provider did not obtain a complete sworn disclosure statement (SDS) from a new employee prior to the first day of employment. Evidence: 1. The provider stated that Caregiver # 1's first day of employment was the first Monday of November which was 11/1/21. 2. Caregiver # 1's SDS was dated 11/16/21.
Plan of Correction:	In the future I will get a complete SDS prior to first day of employment.
Standard #:	63.2-1720.1-B-2
Description:	Based on staff interview, the provider did not obtain results of a finger print based criminal history check for an employee prior to the first day of employment. Evidence: 1. The provider stated that Caregiver # 1's first day of employment was 11/1/21. 2. The provider stated that she had not obtained the fingerprint results for Caregiver # 1 yet.
Plan of Correction:	The fingerprint results were obtained on 12/2/21.
