Comments:
An unannounced monitoring inspection was conducted on 11/29/21 from 9:55am to 12:15pm with the provider and one assistant. There were six children in care, ranging from six months to three years of age, for a total of 15 points. Children were observed watching a movie, reading a book with the provider, having free play with toys, and washing hands. Five child records and two staff records were reviewed. The provider and assistant have current certification in CPR and First Aid. No medications were reported to be onsite and the provider is aware that Medication Administration Training (MAT) must be obtained in order to give medication to enrolled children. The first aid kit was observed. The emergency drill log, attendance, and menu were also reviewed. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of five child records and interview with the provider, the provider did not obtain documentation of a physical examination for a child by a physician within 30 days of attendance. Evidence: 1. The record for Child B, with a start date of 8/23/21, did not contain documentation of a physical. 2. The record for Child C, with a start date of 6/14/21, contained documentation of a religious exemption for the physical that was dated 8/5/21.

Plan of Correction: Child B has an appointment scheduled for 12/15/21 and the parent will provide me with a copy. Moving forward I will make sure that all documentation for physicals are obtained within 30 days of attendance.

Standard #: 22VAC40-111-50-A
Description: Based on record review and interview with the provider, the provider did not ensure that all children were included on the record of attendance each day. Evidence: 1. At approximately 10:15am on 11/29/21 the daily attendance record had documentation of five children being present. There were six children present at the time. 2. Interview revealed that the child not included on the attendance had arrived at approximately 8:45am.

Plan of Correction: We will ensure that all children are added in a timely manner moving forward.

Standard #: 22VAC40-111-60-B
Description: Based on review of five child records and interview with the provider, the provider did not ensure that each child's record contained all of the required documentation. Evidence: The record for Child A did not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: We have requested this information from the parent and will speak with them again about the requirement for two emergency contacts.

Standard #: 22VAC40-111-90-A
Description: Based on review of five child records and interview with the provider, the provider did not obtain documentation that a child had been adequately immunized before attending. Evidence: The record for Child A, with a start date of 11/17/21, did not contain immunizations.

Plan of Correction: Immunizations were obtained on 11/30/21. Moving forward we will ensure that we obtain immunizations before children start.
