Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an on-site tour of the program.
A renewal inspection was initiated on December 16, 2021 and concluded on December 16, 2021. An onsite inspection was conducted. There were 12 children present, ranging in ages from five months to four years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and one provider and one household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider did not ensure that all areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable conditions.
Evidence:
1. The wooden play structure had a broken floor board creating a tripping hazard. The floor board was cracked and splintered creating a hazard that could snag skin. The play structure had two missing balusters from the raised platform.

Description: Based on observation, the provider did not ensure hanging items were kept out of reach of children under five years of age.
Evidence:
In room #4 there was a camera cord unplugged hanging beside a crib. There was a child napping in the crib within reach of the camera?s cord.
