This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, document review and an interview with the provider.
A monitoring inspection was initiated on 1/24/2022 and concluded on 1/25/2022 with an exit interview. The provider was contacted by telephone and a virtual inspection was conducted. There was one child present, age seven, with the provider supervising. The inspector reviewed compliance in the areas of administration, household members, physical health of caregivers and household members, caregiver training, physical environment and equipment, care of children, preventing the spread of disease, medication administration, emergencies, transportation and nutrition. A total of one child record and three caregiver/household member records were reviewed.
The information gathered during the inspection determined non-compliance with one applicable standard or law and the violation is documented on the violation notice issued to the program. Standard #: 8VAC20-800-90-C Description: Based on review of documentation, and an interview with the provider, documentation of additional immunizations were not obtained as required. Evidence: the record for child #1 (date of first attendance: 12/1/2014) did not contain a record of any immunizations that were administered after 2014. Plan of Correction: Provider stated she will obtain an updated immunization record for the child.
