Comments:
An unannounced renewal inspection was conducted today between the hours of approximately 9:45 am through 11:48 am. There were 5 children (10 points) with caregivers. 5 children's files and 3 staff/household member's files were reviewed.

The children were engaged in the following: breakfast, toy play, circle time, bathroom breaks, hand washing, and other organized activities. Interactions between the children and staff were positive. One crib, the physical space of the care area and the family home, emergency supplies, evacuation drills, attendance records, injury reports, and the Emergency Preparedness Plan were reviewed. There were no medications on site today.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions regarding this inspection, please contact the Licensing Inspector, Maria Robles at maria.robles@doe.virginia.gov.

Violations:
Standard #: 8VAC20-800-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #3 did not contain the policy number of the child's medical insurance.

Plan of Correction: I have requested it from the parents. Child #3 will disenroll soon so this number won't be needed.

Standard #: 8VAC20-800-280
Description: Based on observation and interview with the provider, the provider did not ensure that potentially poisonous substances, materials and supplies such as cleaning agents, disinfectants, and deodorizers were stored away in areas inaccessible to children.
Evidence:
1) In the bathroom of the care area, there was an air freshener spray bottle in the shower inside a bucket on the floor ,and there was a bleach bottle and glass cleaner spray bottle inside the cabinet under the sink. The cabinet doors were not locked. 2) The provider confirmed the location of these cleaning agents and deodorizer.

Plan of Correction: I will put a lock on the cabinet door today.

Standard #: 8VAC20-800-320-B
Description: Based on observation and interview with the provider, child-resistant protective covers were not installed on three unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence:
1) One electrical outlet behind the book shelf to the right of the reading area and two electrical outlets behind the cabinet to the left of the reading area did not have protective covers. 2) The provider confirmed that these electrical outlets did not have protective covers.

Plan of Correction: Corrected during the inspection.

Standard #: 8VAC20-800-320-G
Description: Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) The most recent fireplace inspection was dated 08/18/2020.

Plan of Correction: I have contacted the company to come conduct an inspection.
