Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews. A monitoring inspection was initiated on 1/21/22 and concluded on 1/27/22. The provider was contacted by telephone and a virtual inspection was conducted. There were 6 children present, ranging in ages from 1 year to 4 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 2 staff records, and 2 household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on Thursday 2/3/22. Description:	Based on record review, the provider did not ensure that staff submit to a fingerprint based criminal history background check prior to employment. Evidence: Staff #2 (Date of Hire: 1/10/22) does not have fingerprinting results on file.
Plan of Correction:	Next time, I will submit to a Fingerprint Criminal Background prior to employment.

Description:	Based on record review, the provider did not obtain a signed Sworn Disclosure Statement for staff prior to employment. Evidence: Staff #2 (Date of Hire: 1/10/22) had a signed Sworn Disclosure Statement on file completed 1/20/22.
