Technical Assistance:
Effective September 1, 2021 all child day programs operating in a building/home build before 2015 are required to be equipped with at least one carbon monoxide detector.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 01/15/2022 and concluded on 02/03/2022. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present with two caregivers. The ages of the children in care counted for a total of 19 points. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's records and 2 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on record review and interview, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child 1, enrollment date 07/12/2021, lacks documentation of a physical exam.
2. Child 2, enrollment date 09/13/2021, lacks documentation of a physical exam.
3. The provider confirmed that a documented physical exam was not on file for child 1 and child 2.

Plan of Correction: Each parent will be asked to provide a current physical exam within 30 days.

Standard #: 8VAC20-800-90-C
Description: Based on record review and interview, the provider failed to obtain documentation of immunizations once every six months for children under the age of two years.

Evidence:
1. The most recent immunizations on file for child 1 were administered on 07/12/2021.
a. Child 1 is under the age of two years.
2. The provider confirmed that an updated immunization record had not been obtained for child 1.

Plan of Correction: The parent of child 1 will be asked to provide an updated immunization record.

Standard #: 8VAC20-800-210-A
Description: Based on record review and interview, the provide failed to ensure that caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1. Caregiver 2 obtained only 12 of the required 16 hours of annual training for 2021.
2. The provider confirmed that caregiver 2 did not obtain all required training hours for 2021.

Plan of Correction: Caregiver 2 has obtained 2.5 hours of training in 2022 and will obtain all 16 hours by the end of 2022.
