An unannounced renewal inspection was conducted with the provider beginning at11:32 AM and ending at 1:09 PM. There were 7 children in the care of the provider and assistant. The children were ages 3 months, 10 months, 12 months, 23 month, 23 months, 4 , 5 for a total of 17 points. The children were observed finishing lunch, playing with toys, and coloring. Five children's records were reviewed. Three pet records were reviewed and in compliance. Three household records were reviewed and in compliance. There were no medication on site to be reviewed. The first aid kit, flashlight and weather-band radio were observed and in compliance.

Standard #: 8VAC20-800-100-A Description: Based on a review of five children's records, not all of the children enrolled had documentation of a physical examination within 30 days of enrollment. Evidence: The records for Child A (start date 8/10/2021), Child B (start date 9/16/2021), Child C (start date: 7/9/2019), Child D (6/21/2021), and Child E (11/21/2020) were missing documentation of a physical examination. Plan of Correction: The provider stated that she would request a copy of the physical examination from the parents.
Standard #: 8VAC20-800-90-A Description: Based on a review of five children's records, not all of the children enrolled had documentation of a immunization on the first day of enrollment. Evidence: The records for Child B (start date 9/16/2021) and Child E (11/21/2020) were missing documentation of a immunization. Plan of Correction: The provider will request a copy of the immunization record for the children.
