Technical Assistance:
Technical assistance was offered regarding background checks and attendance records.

Comments:
A renewal inspection was initiated on 2/16/22 and concluded on 2/22/22. The inspection was held in person, from 11:00 am to 11:30 am. There were three children (8 points) in direct care and supervision of one caregiver. The children were observed playing with toys. A walk through of the family day home was conducted to assess the indoor and outdoor space used for children. The provider maintained current emergency drill records and first aid kit. There were no medications on site. 2 children's records and 1 caregiver's record was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-800-60-B
Description: Based on records review, children's records did not contain complete information.
Evidence: Child # 1's record contained the information to parents form. Information that was required to be listed on Page 1 of this form, was not completed by the provider.

Plan of Correction: Form Completed.

Standard #: 8VAC20-800-240-A
Description: Based on observation, furnishings outside of the family day home were not maintained in a safe and operable condition.
Evidence:
1. The metal see saw on the outdoor front yard was covered in rust.
2. The metal climbing dome on the outdoor front yard had areas that were rusty.
3. The plastic picnic bench on the outdoor front yard was broken on the top, creating an injury hazard for the children.

Plan of Correction: 1. Metal see saw has been thrown away (2/25/22). 2. Will paint dome in 30 days. 3. Picnic bench has bee thrown away.(2/25/22)

Standard #: 8VAC20-800-260-C
Description: Based on observation, portable wading pools when not in use during the family day home's hours of operation, were not emptied, sanitized, and stored in a position to keep them clean and dry.
Evidence: A large wading pool sitting behind two playhouses on the outdoor front yard, was filled with standing water.

Plan of Correction: Water was removed/dumped out and the pool was trashed.

Standard #: 8VAC20-800-500-D
Description: Based on observation, outdoor equipment did not meet all requirements.
Evidence: The "S" hooks for the swings located on the front play area were not tightly closed. There was a gap of more than the thickness of a penny for each hook.

Plan of Correction: The gap was closed.
