This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A focused monitoring inspection was initiated on Friday, March 11, 2022 and concluded on Monday, March 14, 2022.The provider was contacted by telephone and a virtual inspection was conducted. There were two children totaling 8 points present with 1 caregiver providing supervision. The
inspector reviewed compliance in the areas of administration, physical environment, personnel, care of children, preventing spread of diseases, emergencies, nutrition. There was 1 child record and 1 caregiver record reviewed for this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-60-B
Description: Based on review of child's record, the provider did not ensure to obtain all required information for each child.

Evidence: In Child#1's record (start date:11/1/2021), the name, address, and telephone number of two designated persons to contact in case of an emergency included one of the child's parent. Therefore, only one designated person to contact was provided if the parent cannot be reached.

Plan of Correction: The provider will ensure to obtain a second designated person to contact in case of an emergency from child's parent.
