Technical Assistance:
Discussed TB Screenings have a 2 year span. Also discussed items needed for Subsidy Services if/when interested as well as Zoning requirements to increase Capacity.

Comments:
An unannounced renewal inspection was conducted in person today. The Inspector arrived to observe 9 children with 3 caregivers for a total of 22 points. The children were observed handwashing before lunch and caregivers were observed interacting with the children during lunch. The Provider has a large supply of age stage equipment and materials available for the children. The Provider and a caregiver are CPR certified and the Provider is MAT certified but stated there is no one currently on medication. The Inspector reviewed records, documentation of drills, menu, and noted the Provider has attended over 30 hours in annual training. Please call with questions 703-479-4679

Standard #: 22.1-289.036-E
Description: Based on record review, it was determined the Provider employed a person to help in caring of children prior to first obtaining a Background check.
Evidence:
1. Staff # 3 started in August, 2021, but the Finger Print Search was not completed until November, 2021

Plan of Correction: I got confused about that but I will make sure all future staff have background check before they start.

Standard #: 8VAC20-800-60-A
Description: Based on record review of 3 children's files, it was determined the Provider did not maintain 2 files with all required information.
Evidence:
1. Child #1 was missing an emergency contact.
2. Child #3's file indicated a child had been present more than a year and the annual review- by parent for information being current was not documented.

Plan of Correction: We will ask again for an emergency contact and we will have parents review forms annually.

Standard #: 8VAC20-800-70-A
Description: Based on record review it was determined the Provider did not provider 2 parents, in writing,
that the home is in Presence of a pet and that Amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider.

Plan of Correction: We will make sure all parents have the written acknowledgment form that contains this information.

Standard #: 8VAC20-800-210-A
Description: Based on interview and record review, it was determined a caregiver did not complete 16 hours in annaul training.
Evidence:
1. Caregiver #2 stated she was in the process of taking a training but has not taken the 16 hours in annual training.

Plan of Correction: I will make sure the Caregiver takes training this year.
