Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A monitoring inspection was conducted on 4/1/2022. The provider and 3 assistant caregivers were caring for 12 children, totaling 31 points. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 4 children's records and 3 caregiver records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-700-A
Description: Based on a review of children's medications, it was determined that one prescription medication was on site without all required documentation.
Evidence:
One child's long term prescription medication was on site without written authorization from the parent.

Plan of Correction: I will ask the parent to complete the medication authorization form.

Standard #: 8VAC20-800-180-A
Description: Based on a review of caregiver records, it was determined the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening.
Evidence:
The TB Health Screening for caregiver C expired on 12/12/21.

Plan of Correction: I will make sure she gets a new TB screening completed next week.
