Technical Assistance:
1. Discussed behavioral challenges of a child enrolled. Inspector will contact appropriate agencies for screening and testing services, and training resources for the provider. Inspector will provide parent with points of contact at local agencies.

Comments:
An unannounced monitoring inspection was conducted at 1:40pm. There was 1 child present = 2 points with the provider. Ratios and supervision were in compliance at the time of inspection. The child was observed playing with toys and eating snack. The home was well supplied with toys and equipment for children, and appeared clean and organized today. Information gathered during the inspection determined there was an area of non-compliance with applicable standards or laws reviewed. Violations:
Standard #: 8VAC20-800-100-A
Description: Based on records reviewed, it was determined that 1 of 2 children's record did not include a physical exam within 30-days of the first day of attendance. Evidence: Child #9 - There was no physical exam on-file. The child's 1st day of attendance was 11/1/21.

Plan of Correction: The Provider requested the physical exam and was told by the parent that the doctor would not complete the physical exam form. The Provider will request again from the parent.
