Comments:

A renewal inspection was conducted on April 13, 2022. There were 7 children present, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Standard #:	22.1-289.035-B-2
Description:	Based on review of 3 caregiver records, 1 did not submit to fingerprinting. Evidence Caregiver #3 did not submit to fingerprinting before their first day of employment.
Plan of Correction:	She will make an appointment and get it done.
Standard #:	8VAC20-770-60-C-2
Description:	Based on review of 3 caregiver records, 3 do not have a current central registry finding. Based on review of 1 household member record, 1 does not have a current central registry finding within 30 days of turning 14 years. Evidence The record for caregiver #1 and #2 have expired central registry findings The record for caregiver #3 does not have any central registry finding within 30 days of first day of employment.
Plan of Correction:	I am trying to submit the forms using the portal and having difficulty. I will send them in the mail if I cannot use the portal. I will send my inspector the results when they are received.
Standard #:	8VAC20-800-60-B
Description:	Based on review of 5 children records, 1 record did not contain the required information. Evidence The record for child #2 did not contain an address for one emergency contact, and no information for a second emergency contact.
Plan of Correction:	I will have the parents get the emergency contact information and make sure I always have two when they start.
Standard #:	8VAC20-800-90-C
Description:	Based on review of 5 children records, 2 records for children under the age of 2 did not have additional immunizations every six months. Evidence The records for child #1 and #3 (both under the age of two) did not have additional immunization records every six months.
Plan of Correction:	I will get a copy and put it in their record. I will make sure to get updated every six months.
