Description: Based on observations, it was determined that all infant bottles were not labeled with the child's full name and the date.

Plan of Correction: The bottles will labeled correctly today.

Standard #: 8VAC20-800-120-B
Description: Based on a reveiw of caregiver records, it was determined the files for each assistant/substitute caregiver do not contain all required information.
Evidence:
1 - The file for assistant caregiver A does not include the information for an emergency contact person.
2 - The file for the substitute caregiver does not include documentation of qualifications.

Plan of Correction: Both files will be updated immediately.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, it was determined that two records do not contain all required information.
Evidence:
1 - Child record B does not include the parent's medical insurance name and policy number.
2 - Child record C does not include the policy number for the parent's medical insurance.

Plan of Correction: Both parents will be asked for the information.

Standard #: 8VAC20-800-690-D
Description: Based on provider interviews and observations of the child care space, it was determined that the diapering surface was not cleaned and sanitized after each use as required.
Evidence:
It was reported to the Inspector that the diaper changing pad is cleaned with pre-packaged cleaning wipes in between each use. This method does not clean and sanitize the diapering surface as required.

Plan of Correction: I will use a spray bottle of bleach/water solution and soap/water solution in between each diaper change.
