Technical Assistance:
Reviewed food preparation in order to reduce the risk of choking hazards. The provider reported that all foods are prepared as required.

Comments:
A monitoring inspection was conducted on 4/26/2022. The provider was caring for 4 children, totaling 12 points. This inspection took place in the middle of the day. The children were observed eating lunch and preparing for their afternoon nap.
The Inspector reviewed compliance in the areas of administrative record keeping, health records, the physical plant, emergency preparedness, nutrition, and caregiver training records.

Violations:
Standard #: 8VAC20-800-870-E
Description: Based on observations, it was determined that one child ate food that was not obtained in a sanitary manner.
Evidence:
The Inspector observed one 12 month old child crawl beneath the older children's lunch table and eat two items of food from the floor.

Plan of Correction: I was distracted when this happened. It will not happen again.

Standard #: 8VAC20-800-880
Description: Based on observations of lunch service, it was determined the provider did not follow the most recent, age-appropriate nutritional requirements recognized by USDA.
Evidence:
The children were served a bowl of noodle soup for lunch.

Plan of Correction: The soup is flavored with chicken and vegetable flavorings. I added a banana during the inspection. I will make sure a full balanced lunch is always served.

Standard #: 8VAC20-800-60-B
Description: Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
Child record B does not include the mother's employment contact information; the name/address/phone number for two emergency contacts.

Plan of Correction: The parent will be asked to complete the missing information.

Standard #: 8VAC20-800-210-A
Description: Based on a review of provider training documents, it was determined that she does not hold current satisfactory completion of First Aid and CPR.
Evidence:
The provider's First Aid and CPR certification expired on 3/7/2022.

Plan of Correction: I will make it a priority to renew my certification this week.

Standard #: 8VAC20-800-500-C
Description: Based on observations, it was determined that two pieces of outdoor play equipment have been placed over hard surfaces.
Evidence:
1 - The backyard slide has been placed over top of a hard dirt and grass surface.
2 - The backyard swing has been placed over top of a surface of hard bricks.

Plan of Correction: I will put absorbent rubber mats under the slide.
I will move the swing to a safer place.

Standard #: 8VAC20-800-520-A
Description: Based on observations and provider interviews, it was determined that although cribs are provided the provider does not use a crib for each child under the age of 12 months.
Evidence:
It was reported to the Inspector that one 8 month old infant is permitted to sleep in a bouncy seat on the floor next to the provider. This child is not moved to a crib for each nap.

Plan of Correction: The child does not like sleeping in the crib, but I will begin moving her to the crib for all naps.

Standard #: 8VAC20-800-690-D
Description: Based on observations, it was determined the diapering surface is not cleaned and sanitized between each use as required.
Evidence:
It was reported to the Inspector that the diaper changing mat is wiped down with disposable cleaning wipes and sprayed with disinfecting air freshener. However, the diaper changing mat is not cleaned and sanitized with approved cleaners and/or solutions as required.

Plan of Correction: I will go back to cleaning the mat with soap/water then bleach/water solutions.

Standard #: 8VAC20-800-690-E
Description: Based on observations, it was determined that one soiled disposable diaper was not disposed of in a leak-proof or plastic-lined storage system.
Evidence:
On the floor of the children's bathroom, the Inspector observed one soiled diaper wrapped inside a soiled glove. This diaper had not been disposed of inside the diaper trash can.

Plan of Correction: I will make sure the diapers are always immediately thrown into the diaper trash can.
