Comments:
A monitoring inspection was conducted on April 26, 2022. There were four children present, one of which is a household member, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Violations:
Standard #: 8VAC20-800-730-D
Description: Based on observation, provider shall ensure medications are kept in a locked place using a safe locking method that prevents access by children.

Evidence: A prescription medication was observed sitting on the kitchen table.

Plan of Correction: Medication was immediately removed and placed with other medications in a locked cabinet in the master bathroom.

Standard #: 8VAC20-800-50-A
Description: Based on observation, provider did not ensure there is a written record of children in attendance each day.

Evidence: There was not a written record of attendance for the three children present during inspection.

Plan of Correction: Provider will begin again writing down children's attendance each day.

Standard #: 8VAC20-800-60-B
Description: Based on review, provider did not ensure 2 of 2 children's records have documentation of required information.

Evidence: The records for child #1 and child #2 did not have documentation of parent's employment information, physician information, emergency contact information, insurance plan information, parent's signed acknowledgement of receiving a copy of the family day home's emergency preparedness plan and liability insurance declaration.

Plan of Correction: Provider will have parent's complete the additional paperwork that will include all required information. Provider will review the document to ensure it is complete.
