A renewal inspection was initiated and concluded on 03/16/2022. There was 1 child in care and the provider's child, with the provider supervising. The inspector reviewed compliance in the areas of administration, household members, care of children, emergencies and nutrition. A total of 2 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-100-B Description: Based on record review, physical examinations obtained prior to attendance were not conducted within required timeframes for 2 of 2 children.
Evidence:
1. Child #1 began attendance on 2/21/2022. The physical examination in the record for Child #1 (age 5) was conducted on 10/29/2019, more than 12 months prior to attendance.
2. Child #2 began attendance on 2/21/2022. The physical examination in the record for Child #3 (age 4) was conducted on 7/20/2020, more than 12 months prior to attendance.Plan of Correction: I will ask parent for new physical today.
