Two shelter-in-place practice drills are required annually.

Comments:
An unannounced monitoring inspection was conducted on 05/12/2022 from 10:04 am - 11:10 am. At the time of entrance there were two children in care with the provider. The ages of the children counted for a total of four points. Children were observed during morning program time which included a matching game, music and completion of bubble painting. Records were reviewed for the two children in care, the provider and one household member.

Violations were found in the area of administration, physical environment and required postings. These violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 8VAC20-800-60-A
Description: Based on record review, the provider failed to ensure that children's records contain all required elements.

Evidence:
1. The enrollment record for child 1 lacked the home address and place of employment for one parent.

Plan of Correction: The provider stated:
The parent has been out of the country until last week.
The parent of child 1 will be asked to provide this information at pick-up today.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition.

Evidence:
1. Four screws were observed protruding from the back of the fence panel play board where the play objects have been attached.
a. The screws have a sharp point and could injure a child's finger if touched.

Plan of Correction: The provider stated:
These screws had been covered however, they must have come off with the rain.
New coverings will be placed over the tips of each exposed screw to close up the sharp point.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation, the provider failed to ensure that the findings of the most recent inspection of the facility shall be posted.

Evidence:
The findings from the most recent monitoring inspection, conducted 10/22/2021, were not posted.

Plan of Correction: The provider stated the inspection summary had been posted but was taken down recently when the parent board was updated.
Going forward, the inspection reports will remain posted.
