Technical Assistance was discussed with the provider regarding:
-out of state background checks
-attendance
-parents annual sign-off of child records
-orientation for new caregivers

Comments:
An unannounced renewal inspection was conducted on 5/12/22 from 10:30am to 12:05pm with the provider and two assistants. There were 10 children in care, ranging in age from 14-months to five-years-old, for a total of 22 points. Children were observed cleaning up toys, practicing letters and sounds, having a small snack, washing hands, playing with table-top toys, and dancing and singing. Three child records, three staff records, and two pet records were reviewed. The provider and both assistants have current certification in CPR and First Aid. The provider reported that there was no medication on site for children and she understands that she must obtain certification in Medication Administration Training (MAT) in order to give medication. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/18/22. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Standard #: 22.1-289.035-A
Description: Based on review of three caregiver records, the provider did not obtain the results of a search of the central registry for a caregiver every five years from the date of the last check. Evidence: The central registry on record for Caregiver B was dated 4/21/17.

Plan of Correction: Will order CPS for Florida and New York for new employee.

Standard #: 8VAC20-800-90-C
Description: Based on review of three child records, the provider did not obtain documentation of additional immunizations every six months for children under the age of two years. Evidence: 1. Child A was enrolled when they were two-months-old in 2019. The immunizations on record were from 2019 when the child was enrolled. 2. Child B was enrolled when they were six-months-old in 2021. The immunizations on record for Child B were from 2021 when the child was enrolled. 3. Child C was enrolled when they were four-months-old in 2019. The immunizations on record were from 2019 when the child was enrolled.

Plan of Correction: Child are up to date with immunizations as of 6/2/2022.

Standard #: 8VAC20-800-200-C
Description: Based on review of three caregiver records, the provider did not ensure that documentation of orientation was signed and dated by the provider and assistant. Evidence: The record for Caregiver A did not contain documentation of orientation.

Plan of Correction: Orientation was provided on 5/12/2022.
