A monitoring inspection was conducted on May 16, 2022. There were four children present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-800-60-B Description: Based on review, provider did not ensure 2 of 2 child records have documentation of required information.
Evidence: The record for child #1 did not have father?s employment information and the record for child #2 did not have mother?s employment phone number, 1st emergency contact address and a 2nd emergency contact.Plan of Correction: Provider will request information from parents and information will be added to the records.
Standard #: 8VAC20-800-430-B Description: Based on observation, provider did not ensure pressure mounted gates are not used as protective barriers at stair openings.
Evidence: A pressure-mounted gate was observed at the bottom of the stairway opening leading to the second floor.Plan of Correction: A larger pressure-mounted gate will be secured in the hallway preventing children access to the stairway.
