Comments:
A unannounced renewal inspection was conducted on May 5th, 2022. There were 7 children present (26 points) with 2 caregivers. The home, children's records, staff records, evacuation records, cribs, first aid kits, outside areas and bathrooms were reviewed. The children were observed playing with floor toys or being bottle fed. Areas of non-compliance were cited on the violation notice. Standard #: 8VAC20-770-40-D-4-a
Description: Based on a review of household member records, the provider failed to obtain a current Criminal History record check.

Evidence:
Household member#1's record does not contain documentation of a current Criminal History record check.

Plan of Correction: I will have household member's paperwork sent in tomorrow - 05/06/2022.

Standard #: 8VAC20-800-700-A
Description: Based on a review of child records, the provider failed to obtain a written authorization from a parent to administer a medication for a child in care.

Evidence:
Child # 4 has an Auvi-q epinephrine injector at the FDH in case of an emergency due to a food allergy. There is no written authorization.

Plan of Correction: I will get written authorization from parent.

Standard #: 8VAC20-800-760-A
Description: Based on observation, the provider failed to ensure that the first aid kit contained all required supplies.

Evidence:
The first aid kit was missing a first aid manual.

Plan of Correction: I will purchase a first aid manual and put it in the first aid kit.

Standard #: 8VAC20-800-830-B
Description: Based on review of evacuation log, the provider failed to ensure that shelter-in-place procedures will be practiced a minimum of twice per year.

Evidence:
The provider did not have documentation of shelter-in-place practice drill twice a year.

Plan of Correction: I will practice shelter-in-place drills at least two times each year.

Description: Based on review of child records, the provider failed to obtain documentation of a physical exam by or under the director of a physician prior to c child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1's record does not contain documentation of a physical examination by the 30th day of attendance. First day of attendance - 09/07/2021

Plan of Correction: I will ask the parent for the physical form and place in the child's file.

Standard #: 8VAC20-800-60-A
Description: Based on a review of children's records, the provider failed to obtain and maintain all required documents for each child enrolled.

Evidence:
Child #2's record did not contain documentation of parent's work address, liability insurance, medication policy.
Child #4's record did not contain documentation of parent's work phone numbers.
Child #5's record did not contain documentation of the parent agreement or the medication policy.

Plan of Correction: I will ask parents for records and place them in child files.

Standard #: 8VAC20-800-90-A
Description: Based on a review of child records, the provider failed to obtain documentation that the child has been adequately immunized according to requirements of 32.1-46 of the Code of Virginia and applicable State Board of Health regulations, by the first day of attendance.

Evidence:
Child #1's record does not have documentation of immunizations by the first day of attendance. First day of attendance was 09/07/2021.

Plan of Correction: I will request immunization record from parent and place in child's file.

Standard #: 8VAC20-800-180-A
Description: Based on review of staff and household member records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form in accordance with the requirements, every two years from the date of the first screening.

Evidence:
Household Member #1 and #2's records do no have documentation of a current Tuberculosis Screening.

Plan of Correction: I will make sure household members get TB test or screening as soon as possible.

Standard #: 8VAC20-800-210-A
Description: Based on a review of caregiver records, caregivers failed to obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
Caregiver #1 and #2's records did not have documentation of 16 hours of relevant training.

Plan of Correction: I will make sure caregiver take 16 hours of training each year.

Standard #: 8VAC20-800-220-A
Description: Based on review of caregiver records, the provider failed to ensure that whenever the family day home has a agreed to administer prescription medications or nonprescription medications, the administration shall be performed by a caregiver who has satisfactorily completed a training program for this purpose developed or approved b the Board of Nursing and taught by a registered nurse, licensed practical nurse, doctor of medicine or osteopathic medicine or pharmacist.

Evidence:
Caregiver #1 and Caregiver #2's records did not have documentation of Medication Administration Training.

Plan of Correction: Caregivers will get certification in medication administration as soon as possible.
