Comments:
An unannounced renewal inspection was conducted on-site on May 26, 2022. The provider was available during the inspection. There were 3 children present, ranging in ages from 9 months to 2 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-800-100-A
Description: Based on a review of children's record and interview on May 26, 2022, the provider did not ensure to obtain a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.
Evidence: The record of child #3 (enrolled 3/28/22) did not contain documentation of a physical exam. The provider acknowledged the physical was missing.

Plan of Correction: I have talked with the parent about the physical exam and inform them to call the doctor today to get a copy of the document. Never let the child start daycare without the physical form being filled out.

Standard #: 8VAC20-800-110-B
Description: Based on a review of children's records and interview on May 26, 2022, the provider did not ensure that immunizations received shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
Evidence: The record of child #3 (enrolled 3/28/22) contained documentation of immunizations that were not signed by a physician, the physician's designee, or an official of a local health department. The provider acknowledged the immunizations weren't signed.

Plan of Correction: Always check for signatures.

Standard #: 8VAC20-800-280
Description: Based on observation on May 26, 2022, the provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.
Evidence: There was a Lysol spray on a recliner in the area children are cared for. There was a bag in the hallway, where children are cared for, that contained several spray bottles of Lysol. The label on the Lysol spray contained the language "keep out of reach of children" and "caution."

Plan of Correction: I have removed all poisonous materials out of the hallway and in the chair. Place one in the car and place the other up high.

Standard #: 8VAC20-800-350-E
Description: Based on observation, the provider did not ensure that hot water taps available to children were maintained within a range of 105?F to 120?F.
Evidence: The hot water temperature in the children's bathroom was obtained using a digital thermometer. The hot water temperature registered at 125.3?.

Plan of Correction: I have talked to the electrician to come by today to change the temperature of the water. Have the electrician to come once a year to check and/or maintain the temperature of the water.
