Comments:
An unannounced renewal inspection was conducted today, 5/28/2022.
There was one child in care totally of two points with one caregiver.
A review of child's record, providers record and household members records was conducted.
The child played with the available toys and had a snack during the inspection.
Inspection began 10:00 am ended 11:30 am.

Standard #: 8VAC20-800-830-A
Description: Based on review of records and interviews the the provider failed to practice emergency evacuation drills monthly with children that are in care.
Evidence:
There have been no emergency evacuation drills practiced February -May, 2022.

Plan of Correction: Will be doing practice emergency drills from now on effective

Standard #: 8VAC20-800-60-B
Description: Based on review of children's records the provider failed to ensure that the record contained the following information.
Evidence:
Child record #1 did not have the parent's phone number, two emergency contacts and names of persons authorized to pick up the child.

Plan of Correction: Provider has spoken w/ guardian of the child. I have updated that information. [sic]

Standard #: 8VAC20-800-80-A
Description: Based on review of records and interviews the provider failed to obtain from the parent within seven business days of the child's first day of attendance verification of identity and age of the child.
Evidence:
The first day of attendance for child #1 was 2/5/2022. At the time of the inspection there was no verification of identity and age in the record.

Plan of Correction: Child's record form has been updated [sic]

Standard #: 8VAC20-800-80-D
Description: Based on review of children's records the provider failed to notify the local law-enforcement agency if the parent does not provide verification of identity and age within seven business days of the child's first day of attendance.
The first day of attendance for child #1 was 2/5/2022 and the local law-enforcement agency had not been notified that the parent did not provide a proof of identity and age.

Plan of Correction: Child's record form has been updated. Child's record form has been updated w/copy of court order. [sic]

Standard #: 8VAC20-800-350-E
Description: Based on observation the provider failed to ensure that the hot water at taps available to to children shall be maintained within a range of 105 F to 120 F.
Evidence:
The water temperature in the sink in the bathroom used by the children measured to 132 F.

Plan of Correction: Hot water heater has been adjusted to 120F.
