Technical Assistance:
Reminded provider that the MAT training has expired, if she is not renewing it please complete new medication administration policy forms for each child.

Comments:
An unannounced monitoring inspection was conducted today from 10:50am-12:20pm. There were 13 children (33 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, napping and playing outdoors. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-40-A
Description: Based on direct observation and record review, the family day home did not ensure that the total number of children receiving care at one time does not exceed the maximum licensed capacity of the home.

Evidence:
During the inspection there were 13 children in care. The capacity for the family day home is 12 children.

Plan of Correction: Same as above.

Standard #: 8VAC20-800-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child #4's record contains an immunization record that was last updated on 8/28/2021.

Plan of Correction: Parent sent over new shot records.

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, the provider does not have a landline telephone.

Evidence:
The provider does no longer have an operable landline telephone in the home.

Plan of Correction: New phone has been ordered and will be installed any time between 6/10-6/15.

Standard #: 8VAC20-800-570-A
Description: Based on observation and interviews, the Provider did not ensured that caregivers follow the point system to avoid exceeding 16 points per caregiver.

Evidence:
On the day of the inspection from 10:50am- 11:30am, there were two caregivers with 13 (non-resident) children in care, totaling 33 points.

Plan of Correction: A neighbor asked for one time favor to watch her kid while she was at the doctor, kid is not enrolled in daycare and it was a one time thing only.

Standard #: 8VAC20-800-690-D
Description: Based on observation, the family day home (FDH) did not maintain a nonabsorbent surface for diapering.

Evidence:
The diaper changing pad had a small hole or tear in the surface, making it absorbent.

Plan of Correction: Purchased a brand new mat the next day.
