Comments:
An initial Subsidy Health and Safety inspection was conducted on Monday, August 5, 2019 from approximately 9:00 AM until 11:00 AM to review health and safety requirements. The caregiver was present and assisted with the inspection. There was one child in care. Based on the child?s age, there was a total of 1 point. During the inspection, the child was observed looking at flash cards, playing with toys and playing on a computer and tablet. All areas in the home used by children were inspected. A walk through of the home was completed with the caregiver. Administrative documentation was reviewed. Three children?s records, one caregiver?s record and one household record were reviewed during the inspection. The provider stated transportation will not be provided and medication will not be administered. Violations of the subsidy health and safety requirements were found. Standard #: 22VAC40-665-170-B-12
Description: Based on documentation review and interview, the caregiver did not ensure a written care plan for one child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction was obtained. Evidence: The record for Child #2 indicated the child has a food allergy. The record did not contain a written care plan, including instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. The caregiver stated she did not have a written allergy plan for Child #2.

Plan of Correction: I can get the written allergy plan today from the doctor.

Standard #: 22VAC40-665-240-I
Description: Based on observation and interview, the caregiver did not ensure a wood burning fireplace was inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence: A wood burning fireplace was located in one of the areas used by the children. The caregiver stated the wood burning fireplace has not been inspected within the last year.

Plan of Correction: I will reach out and get an inspection for the fireplace scheduled.

Description: Based on observation and interview, the caregiver did not ensure the first aid kit included the required supplies. Evidence: The first aid kit given to the licensing inspector did not include a thermometer. The caregiver stated she did not have one in the house.

Plan of Correction: I will buy a thermometer to place in the first aid kit today.

Description: Based on observation and interview, the caregiver did not ensure one working, battery-operated radio was in the home. Evidence: While reviewing the emergency supplies, the caregiver stated she did not have a working, battery-operate radio in the home.

Plan of Correction: I will buy a battery operated radio today.
