Comments:
An unannounced monitoring inspection was conducted on June 16, 2022 to determine compliance with licensing standards. The inspector arrived at 11:25 AM and concluded the inspection at 12:20 PM. Today, there were eleven children, totaling nineteen points, present with 2 caregivers. During the inspection, the children were observed playing with appropriate materials, and sitting in high chairs. Children?s equipment, learning materials, indoor space, kitchen, and restrooms were inspected. Five children's records, one provider record, one assistant record, daily attendance records, fire drill and shelter in place drill logs, and all required postings were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and are documented on the violation notice.

Standard #: 8VAC20-800-730-D
Description: Based on observations conducted, the facility failed to ensure that medications were kept in a locked place. Evidence: 1 medication was stored in an unlocked cabinet in the kitchen.

Plan of Correction: The provider will lock the cabinet.

Standard #: 8VAC20-800-830-A
Description: Based on a review of emergency drills and staff interviews, the provider failed to practice an evacuation drill monthly. Evidence: 1. The provider did not practice any evacuation drills between January 2022-May 2022.

Plan of Correction: The provider will email the drills.

Standard #: 8VAC20-800-60-B
Description: Based on child record review, the provider failed to maintain a record for 1 out of 5 children in care Evidence: 1.Child #5 did not have a record on file.

Plan of Correction: The provider stated that she would let the parent know.

Standard #: 8VAC20-800-90-A
Description: Based on child record review, the provider did not obtain documentation of immunizations prior to enrollment for 1 out of 5 child records Evidence: Child #5 did not have an immunization record on file.

Plan of Correction: The provider will let the parent know.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider failed to ensure that hazardous substances such as cleaning materials, and disinfectants were stored away from food in areas and inaccessible to the children. Evidence: 1. A can of Lysol Disinfectant was accessible to children and stored on a counter. 2. Floor cleaner was accessible in an unlocked cabinet.

Plan of Correction: The provider said that she will put them up

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, the provider did not ensure that there was a landline telephone available, operable, and accessible that does not require electricity to operate. Evidence: The licensing inspector observed no landline telephone in the family day home. The provider stated that she had a new phone but she had not plugged it in yet.

Plan of Correction: The provider will plug in the new phone.
