The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 4/25/2022. Interviews were conducted relating to allegations of transportation and supervision. The preponderance of the evidence gathered during the investigation supports the allegations, so the complaint is determined to be valid. A violation notice was completed by the provider. Standard #: 8VAC20-800-1010 Complaint related: No Description: Based on interviews the provider failed to ensure that during transportation of children, no child is left unattended inside a vehicle.
Evidence:
On 4/7/2022 staff # 1 transported Child #1 (22 months old) and child #2 (3 yr old), to the local health department. Staff #1 parked the car and went inside the building, leaving child #1 and #2 in the car unattended for less than five minutes.Plan of Correction: The car door were locked and the car was not running. Will never leave children in the car again.
Standard #: 8VAC20-800-980-A Complaint related: No Description: Based on record review and interviews the provider failed to obtain from the parent of each child a general written permission allowing the provider to take the children off the premises of the family day home.
Evidence:
On 4/7/2022 staff #1 transported two children that were in car. Child #1 did not have written permission from the parent to be transported.Plan of Correction: Will make sure written permission is always provided before transporting children.
