Standard #: 8VAC20-800-80-A Description: Based on record reviews the provider failed to obtain within seven business days of the child's first day of attendance verification of the child's identity and age.
Child # 2 entered care 10/1/2020 and child #3 entered care 4/18/2019 and there are no documentations of verification of identity or age in the records.Plan of Correction: The provider contacted the parents and the birth certificates were provided.
