Comments:
An unannounced monitoring inspection was conducted on June 21, 2022 from approximately 2:26 pm to 3:34 pm. There were 6 children in attendance. The provider record, substitute provider record, assistant record, household member records, and 6 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected.

Interviews were held with the provider and caregivers throughout the inspection. The inspector interacted with children in care when appropriate. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys to meet the needs of the children. Children were observed taking a nap, eating snack, and playing with toys.

The license and current inspection were posted as well as the drill log. Evacuation drills were conducted monthly. The most recent was on 06/08/22. Shelter in place drill's were conducted two times with the most recent on 06/06/22. The emergency preparedness plan was reviewed and updated annually.

The menu was posted.

Per the provider, medication is not administered.

The center?s first aid kit was inspected and found complete. The home had a working flashlight and battery operated radio.

There were violations cited as a result of this inspection.

Violations:
Standard #: 8VAC20-800-900-B
Description: Based on observation and interview, the provider did not ensure that children are not allowed to eat or drink while walking, running, playing, or lying down. Evidence: 1) A child was sleeping in a crib and began to cry. The provider made a bottle for the child and walked in and handed it to the child and removed an empty bottle that was in the crib. When asked, the provider stated that was how the child liked to fall asleep. 2) The children were served snack while sitting on the living room floor. They were observed to be moving around and playing while eating the snack. When asked, the provider stated that she normally has them sit with trays but forgot to today.

Plan of Correction: Per the Provider "1) I don?t normally do it but I will be sure to never do it again.
2) I will use the trays going forward."

Standard #: 8VAC20-800-140-B-3
Description: Based on a review of three caregiver records, the provider did not ensure that providers and substitute providers have current certification in cardiopulmonary resuscitation (CPR). Evidence: Staff #1 and Staff #2 had CPR that expired in January of 2022.

Plan of Correction: Per the Provider "We will be renewing this weekend."

Description: Based on a review of three caregiver records, the provider did not ensure that providers and substitute providers have current certification in First Aid. Evidence: Staff #1 and Staff #2 had First Aid that expired in January of 2022.

Plan of Correction: Per the Provider "We will be renewing this weekend."

Standard #: 8VAC20-800-240-A
Description: Based on observation and interview, the provider did not ensure that areas of the home, inside and outside, were maintained in a safe and operable condition. Evidence: The back yard contained children?s play equipment that had visible cracks, splinters, breakage, and protruding nails. When asked, the provider stated that she began to tear down the equipment so that she could replace it.

Plan of Correction: Per the Provider "I will not allow the children to play in the back yard until the equipment is removed and replaced."

Standard #: 8VAC20-800-460-D
Description: Based on a review of records, the provider did not ensure to maintain documentation of current rabies vaccination for a one of three dogs in the home. Evidence: Dog #3 had a rabies vaccine that expired 02/10/22.

Plan of Correction: Per the Provider "I will take the dog ASAP."

Standard #: 8VAC20-800-510-A
Description: Based on observation, the provider did not ensure that each child was provided with an individual cot, rest mat, or bed for resting or napping. Evidence: There were 3 children sleeping directly on the living room floor.

Plan of Correction: Per the Provider "I have cots. I will make sure they are on one."

Standard #: 8VAC20-800-530-F
Description: Based on observation, the provider did not ensure that no soft bedding of any kind shall be used under or around infants including but not limited to pillows, quilts, comforters, sheepskins, or stuffed toys. Evidence: Child #1 (18 months) was sleeping in a crib with a pillow and stuffed animals.

Plan of Correction: Per the Provider "I will take them out."
