Comments:
An unannounced monitoring inspection was conducted on July 7, 2022 from approximately 9:30 am to 10:45 am. There were 11 children in attendance for a total of 21 points, with two staff. The provider record, substitute provider record, assistant records, household member records, and 5 children?s records were reviewed. All areas of the home were observed and areas where the children are cared for were inspected.

Interviews were held with the provider and caregivers throughout the inspection. The inspector interacted with children in care when appropriate. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys to meet the needs of the children. Children were observed playing outside, getting ready for water play, practicing writing on worksheets, counting, washing hands, getting diapers changed, and drinking a bottle.

The license was posted. Evacuation drills were conducted monthly. The most recent was on 06/07/22. Shelter in place drill conducted two times with the most recent on 03/31/22. Emergency preparedness plan reviewed and updated annually. The pet dog was up to date with rabies vaccinations.

The menu was posted.

Per the provider, medication is not administered.

The center?s first aid kit was inspected and found complete. The home had a working flashlight and battery operated radio.

There were violations cited as a result of this inspection.

Standard #: 22.1-289.035-B-2
Description: Based on a review of records, the provider did not obtain a national criminal history record check prior to employment for one of five staff. Evidence: The provider did not have fingerprints for Staff #2, start date 06/13/22.

Plan of Correction: Per the Provider ?I thought I had 30 days. I will check for the results.? The provider obtained and emailed verification of the satisfactory criminal record check for Staff #2 to
the inspector on 7/7/2022.

Standard #: 8VAC20-800-330-A
Description: Based on observation and interview, the provider did not ensure that the landline telephone was operable during the hours of operation. Evidence: The provider tried to plug the telephone in and could not get a dial tone. When asked, the provider stated that she ?didn?t know why it wasn?t working.?

Plan of Correction: Per the Provider ?I will call the phone company and see what the problem is. We don?t use it ever so I didn?t know there was a problem.?
