Comments:
An unannounced monitoring inspection was conducted today. There were 3 children (9 points) with one caregiver. The home, children's records, caregiver records, household member records, the Emergency Preparedness Procedures, menus, refrigerator temperatures, changing area, cribs and the first aid kit were reviewed. The children were observed crawling, during diaper change and hand washing. Areas of non-compliance were cited on the Violation Notice. Violations:
Standard #: 8VAC20-800-60-A
Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #3's record does not have documentation of the names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will request the information from the parent and put in child's file.

Standard #: 8VAC20-800-180-A
Description: Based on review of household members records, the provider failed to obtain for each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.

Evidence:
Household Member #3's record does not have documentation of a current tuberculosis test or screening. The last test/screening was 08/30/2018.

Plan of Correction: I will request that HHM get a current TB test.

Standard #: 8VAC20-800-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances are stored in an area inaccessible to children.

Evidence:
There was air freshener on the counter in the bathroom used by the children.

Plan of Correction: I will move the air freshener in the bathroom out of the reach of the children.

Standard #: 8VAC20-800-320-B
Description: Based on observation the provider failed to ensure that child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under 5 years old.

Evidence:
The outlet behind the table in the play room did not have a protective cover.
The outlets in the bathroom used by the children did not have a protective cover.

Plan of Correction: I will purchase additional outlet covers.
The outlet cover in the play room was replaced during the inspection.

Standard #: 8VAC20-820-120-E-1
Description: Based on observation, the provider failed to post required documents.

Evidence:
The provider did not have the findings of the most recent inspection of the facility posted.

Plan of Correction: Provider will post this inspection summary.
