Discussed:
1) Standard 150C ? an assistant 18 years of age or older shall not be left alone with children in care for more than two hours per day.
2) Standard 520A ? cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat or bed. The Virginia Department of Education encourages providers to partner with parents in determining care for children based on their developmental level and needs. The parent and provider are encouraged to assess whether a child is not developmentally ready to be moved out of a crib.
3) Standard 530B ? cribs when being used by infants shall have a tight-fitting bottom cover.
4) Standard 650C ? the provider shall give parents prior notice when a substitute provider will be caring for the children.
5) Standard 690E ? soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic lined storage system that is either foot operated or used in such a way that neither the caregiver?s hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. Diaper genie?s do not meet this standard and are not to be used.

Comments:
An unannounced monitoring inspection was conducted from 10:30 am ? 12:30 pm with the provider. There were four children, ranging from five months to 21 months of age, for a total of 14 points. Two children were observed playing with age-appropriate toys and two children were observed sleeping/resting in cribs. Two children's records and six provider/household member records were reviewed and in compliance. The emergency drill log, daily attendance, annual training and carbon monoxide detector were observed and in compliance. It was reported that there were no medications on site, however the provider has current certification in Medication Administration Training (MAT).

Violations:
Standard #: 8VAC20-800-240-A
Description: Based on observation, not all furnishings of the family day home were maintained in a safe condition. Evidence: there is peeling and chipped paint on both the brown and yellow cribs.

Plan of Correction: Provider stated she will order new cribs or paint them to come into compliance.

Standard #: 8VAC20-800-250-A
Description: Based on observation, the provider failed to ensure that hanging/strangulation hazards were out of reach of children under five years of age. Evidence: a 21 month old child was observed sleeping with a pacifier attached to a ribbon. A sound machine placed in a crib had a hanging cord accessible to children under five years of age in and outside of the crib.

Plan of Correction: Will not use pacifiers with cords when children are sleeping.

Standard #: 8VAC20-800-250-C
Description: Based on observation, the provider failed to ensure items tied across the top of the crib, or toys hung from the sides were removed when the child begins to push up on hands and knees or is five months of age, whichever occurs first. Evidence: a five month old infant was placed to sleep in a crib where there was a mobile hanging from the top and two toys hanging on the side of the crib.

Plan of Correction: Provider removed everything from the crib during the inspection.

Standard #: 8VAC20-800-320-A
Description: Based on observation and interview, the provider failed to ensure that small electrical appliances were unplugged while not in use. Evidence: there were several appliances sitting on the kitchen counter plugged in, and the appliances were not in use.

Plan of Correction: Will ensure all small appliances are unplugged when not in use.

Standard #: 8VAC20-800-510-A
Description: Based on observation, and an interview with the provider, not all children were provided an individual crib for resting/napping. Evidence: five cribs were observed and the provider identified the five children each crib was assigned to. There was no crib assigned or available for an 11-month old infant observed in care.

Plan of Correction: All children will be assigned and have a designated crib or cot to sleep on.

Standard #: 8VAC20-800-530-F
Description: Based on observation, the provider failed to ensure that no soft bedding of any kind was used under or around infants including, but not limited to pillows, quilts and comforters. Evidence: there were multiple blankets in the crib and hanging on the sides of a crib where a five month old infant was placed to sleep. A pillow and two blankets were observed in and hanging on the side of a second crib. A third crib had multiple blankets in the crib.

Plan of Correction: Provider removed everything from the crib during the inspection.
