Comments:
An unannounced monitoring inspection was conducted on 08/08/2022 from 11:00am to 12:20pm. There were 4 children ages 19 months to age 4 years old (9 points) with the Provider. The programming area, outdoor play area, menus, Provider and Household member records, 4 child records, attendance, emergency drills, emergency supplies, and policies were inspected. Provider stated there were no medications onsite. No new staff have been hired since the last inspection. Children were observed playing with puzzles, playing a game, reading a story and eating lunch. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Standard #: 8VAC20-800-780-B
Description: Based on review of four child records, the provider did not conduct an annual review of children?s emergency contact information and the parent?s acknowledgment of the review.

Evidence: The record for Child #2, Child #3 and Child #4 did not contain documentation of an annual review of the children?s emergency contacts, nor did the records contain parent acknowledgment of the review.

Plan of Correction: The documents were signed by 2 parents

Standard #: 8VAC20-800-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances were stored in an area inaccessible to children.

Evidence: Bleach and another generic sanitizer were stored in the bathroom tub and accessible to children.

Plan of Correction: Items were placed in a locked cabinet inside the bathroom
