Comments:
This monitoring inspection was initiated by licensing staff and concluded on 08/25/2022, between the hours of 9:30 am to 11 am. There were 2 children present (4 points), preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of (3) children records, (1) household record, and (1) staff record was reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard #: 8VAC20-800-60-B
Description: Based on review, 2 of 3 children files indicated that not all of the required information and/or documentation was contained within each record.

Evidence -
1. On the date of inspection(08/25/2022), both Child #1, and Child #3 had listed 1 of 2 emergency contact information.
2. The insurance number for the listed medical insurance policy for Child #1 was not available for review.

Plan of Correction: Will get from parents.

Description: Based on review, 1 of 3 children files indicated that not all of the documentation on file was verified by a parent.

Evidence -
1. On the date of inspection (08/25/2022), the Declaration of Liability form given to the parents of Child #1 was returned unsigned.
2. A blank copy of this form was observed in the child's file.

Plan of Correction: Will have parent sign.
