Comments:
An unannounced monitoring inspection was conducted on-site August 23, 2022. The director was available during the inspection. There were 9 children present, ranging in ages from 5 months to 3 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on observation on August 23, 2022, the provider failed to have a working battery-operated weather band radio.
Evidence: The weather band radio was not operational.

Description: Based on a review of the emergency plan on August 23, 2022, the provider failed to review the emergency plan at least annually and update the plan as needed.

Description: Based on a review of children's records on August 23, 2022, the provider failed to obtain a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.
Evidence: The records of child #1 (enrolled 6/1/20), child #2 (enrolled 10/8/21), and child #3 (enrolled 5/10/21) did not contain documentation of a physical exam.

Description: Based on observation and measurement on August 23, 2022, the provider failed to ensure that hot water taps available to children were maintained within a range of 105?F to 120?F.
Evidence: The hot water temperature in the children's bathroom was obtained using a digital thermometer. The hot water temperature registered at 131.9?F.
