Discussed the 4 floor rugs near the door were starting to wear out and will need to be replaced or removed.
- Comments:

An unannounced monitoring inspection was conducted today from 11:43am-12:28pm with the provider on site. There were 3 children with 1 caregiver. A sample of 2 children's files and 2 caregivers/household member's files were reviewed. The children were engaged in the following: watching tv while sitting in high chairs, washing hands, eating lunch and laying in a crib. The physical space, attendance records, menu, drill record and the Emergency Preparedness Plan were reviewed. There were no medications on site. Standard #: 22.1-289.036-B-2 Description: Based on review of household member records, the provider did not obtain the fingerprint results of a household member within 30 days of them turning 18 years old.
Evidence:
1. Household Member #1 turned 18 years of age in April 2022 and the provider did not receive the fingerprint results until 8/19/2022Plan of Correction: I will make sure to obtain when required in the future.
Standard #: 8VAC20-770-40-D-4-a Description: Based on review of household records, the provider did not obtain the sworn statement or affirmation within 30 days of a person turning 18 years of age.
Evidence:
1. Household Member #1 turned 18 years of age in April 2022 and the sworn statement or affirmation was dated 8/19/2022.Plan of Correction: I will make sure to obtain when required in the future.
Standard #: 8VAC20-800-170-A Description: Based on review of household members records, the caregiver did not obtain a current report of tuberculosis screening for each adult household member.
Evidence:
1. Household member #1 became an adult April 2022 and the provider had a tuberculosis screening for the member dated 8/19/2022Plan of Correction: I will make sure to obtain when required in the future.
