Comments:

An unannounced renewal inspection was conducted on 08/30/2022 from 7:00am to 8:00am. There were 3 children ages 9 years old to age 10 years old (2 points) with the Provider and assistant. The programming area, menus, Provider, Assistant and Household member records, 3 child records, attendance, emergency drills, and emergency supplies were inspected. No new staff have been hired since the last inspection. Children were observed watching television, transitioning to go to school and eating snack. Handwashing procedures were also observed. Pet was current with rabies vaccination. Staff were current with certification in CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 9/2/2022. Description:	Based on review of two household member records and interview with the provider, the provider did not have documentation that they obtained a national fingerprint-based criminal history record report for a household members over the age of 18 years old. Evidence: The record for Household member #4 (DOB 10/05/2001) did not contain documentation that a criminal history background check was completed.

Description:	Based on review of the written attendance record and interview with staff, the provider did not ensure they maintained a written record of children in attendance each day. Evidence: 1)During the inspection it was determined there was 1 child present, but the written record reflected there were 0 children present.

Description:	Based on review of staff records and staff interview, the provider did not ensure that all caregivers obtain a minimum of sixteen clock hours of training annually. Evidence: The record for Staff #2 did not contain documentation of the completion of any training hours in the last year. Staff confirmed during inspection that there was no training to report.
