A renewal inspection was conducted Thursday, September 22, 2022. The inspection was initiated at approximately 2:40pm and concluded at approximately 4:15pm. There were nine children present, ranging in ages from 18 months to nine years, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Standard #: 8VAC20-800-60-B Description: Based on review of five children's records, the provider did not ensure one record contained all of the required information.
Evidence:
The record for Child #1 (first date of attendance 08/29/2022) did not contain the name, address and phone number for two people to contact in an emergency when parents cannot be reached.Plan of Correction: I will have the parents provide that information today at pickup.
